Alabama - "An indemnity bond or proof of insurance in the limits of $300,000 for annual permits. Alabama Highway Department shall be listed as Certificate Holder. Must carry minimum insurance of $750/500/10 for all ICC carriers." Alabama also requires a "New Hauler" form to be filled out if you have not hauled in Alabama before. The processing of insurance and the new hauler form generally only takes 2-3 hrs, depending on how busy they are. This must be taken care of before purchasing truck permits.

Arkansas - "Insurance must be on file with the Department. Insurance for individual owner of a mobile home that is up to 12' wide is $25,000/100,000/15,000 or total liability insurance of $115,000. House movers will need a damage bond in addition to the insurance. Insurance for a house is $100,000/300,000/25,000 or total liability insurance of $325,000.
Must carry minimum insurance of $750/500/10 for all ICC carriers." The state accepts faxed copies of insurance certificates. This can be done just before ordering permits.

Delaware - "Insurance must be on file in the amounts of $100/300/50. Must carry minimum insurance of $750/
500/10 for all ICC carriers."

Georgia - "Liability insurance and other security must be maintained during the existence of the permit. Public liability minimum is $300,000 each accident and $100,000 for persons injured, total property damage $50,000. The person receiving the permit promises to hold the Department harmless from such claims and to recompense the Department for damages made by the permitted vehicle or load to a public road. Must carry minimum insurance of $750/500/10 for all ICC carriers. Georgia DOT must be listed as additionally insured on the certificate" Processing the insurance certificate can take 2-4 hours. It is recommended that this is taken care of before ordering truck permits.

Indiana - "Certificate must be on file in minimum amounts of $25,000/50,000/10,000 for personal liability and property damage. Must carry minimum insurance of $750/500/10 for all ICC carriers." The processing of insurance certificates does take 24 - 72 hours. It is highly recommended that this is taken care of before needing to purchase truck permits. If you have not hauled in the state of Indiana before, then you'll also have to apply for an Indiana account number.

Louisiana - "Must have a bond or certificate of insurance on file. $100,000 for one trip or $250,000 for a year. Must carry minimum insurance of $750/500/10 for all ICC carriers." Processing of insurance is done same day, usually within 1-2 hours.

Mississippi - "Applicants must file with the Department a bond or public liability insurance policy endorsement in the amount of $25,000 as security for any damage to the highway. Must carry minimum insurance of $750/500/10 for all ICC carriers. State of MS must be listed as a certificate holder." This process takes up to 24 hours, it is recommended that this is taken care of before purchasing truck permits.

Missouri - "Effective 02/28/2009, no permit will be issued without proof of insurance on file. Must have minimum insurance in the amount of $750,000." This can take up to 4+ hours to process, so it is recommended that this be taken care of prior to ordering truck permits. The state of Missouri must be named as a certificate holder.

Montana - "Minimum of $1,000,000 combined single limit bodily injury and property damage liability per occurrence." A certificate on file with the state is not required, however to purchase a truck permit we need to have your insurance information to give to the state over the phone.

New Mexico - "A certificate of insurance, reflecting the following minimum coverage should be kept in the vehicle
and must be on file with the state. $100,000 automobile bodily injury coverage; $500,000 each person; $100,000 for each accident; automobile property coverage $25,000 each accident. Must carry minimum insurance of $750/
500/10 for all ICC carriers. Insurance Company must be registered with the New Mexico Corporation Commission." The certificate of insurance can be sent in with the application for the truck permit, it is processed immediately and will be kept on file until the insurance expires.

New York - "Liability insurance coverage must be provided to the Department in one of the following ways:

1. A departmental blanket policy provides the state with insurance coverage on each trip permit by payment of the appropriate insurance fee by the permittee.
2. Public liability insurance coverage may be obtained by a permittee and a certificate of protective liability insurance for permits on state highways. Must be filed with the Highway PermitAgent in the main office which provides liability coverage for the people of the state of New York. This certificate must be filed to obtain a monthly, annual or a blanket permit and may be filed for trip or building permit.
3. New York requires a $10,000 - 50,000 bond to be on file for any move if 16' high, 16' wide, 250,000 or over before a permit will be issued. All bonds are held for 30 days before another move can be made.
4. Must carry minimum insurance of $750/500/10 for all ICC carriers.No insurance fee is required if acceptable public liability insurance coverage is on file. The applicant agrees to assume all responsibility and liability for damages to persons or property that may occur during movements of the vehicle through negligence of himself, his agent or employees."

This should be taken care of before ordering permits. The price of the insurance coverage paid for on each single trip is increased if you do not have insurance on file with the state of New York.

North Dakota - "Insurance certificates must be submitted showing proof of liability and property damage coverage
of not less than $300,000. Must have minimum insurance of $750/500/10 for all ICC carriers."

Ohio - "A permit will be issued only after the following proof of financial responsibility has been furnished to the Ohio Department of Transportation Central Permit Office. An endorsement or a surety bond in Confirmation will not be acceptable in lieu of the endorsement. Liability for bodily injury and property damage combined shall not be less than $500,000. Liability for property damage combined shall not be less than $200,000. Tandem trailer combinations - public liability of not less than $500,000 for each person, $2,000,000 for each occurrence for all damages arising from bodily injury, including death, and $1,000,000 for property damage. Must carry minimum insurance of $750/500/10 for all ICC carriers." You MUST have your insurance company fill out an OS 32 form and send it to the state of Ohio. This process can take several days and must be done prior to ordering any truck permits.

Oregon - "Verification of public liability and property damage insurance and or indemnity bond may be required at the discretion and in the amounts determined by the granting authority. Must carry minimum insurance of $750/500/10 for all ICC carriers." You'll need to have this to get your Oregon Carrier File Number (a/k/a OR PUC), but it is a simple process which can be done the same day as ordering your truck permit. To get your PUC# you'll need to call OR DOT directly and have all your insurance information handy.

Pennsylvania - "Minimum coverage for motor carriers and escort vehicles is $1,000,000 combined single limits. Commonwealth of Pennsylvania must be added as an additional insured for property damage and personal injury." This can take up to 3 days to process, it is recommended that this is taken care of before ordering truck permits.

Rhode Island - "In special cases a surety bond is to be furnished to the Department in lieu of adequate liability
insurance. When there is a possibility that a move may damage highway facilities, the applicant will be required to deposit a certified check or other security in an amount determined by the Department. Must carry minimum insurance of $750/500/10 for all ICC carriers."

Tennessee - "(1) Any person, firm, company, corporation or other who undertakes the movement of any overweight
and/or overdimensional article and/or commodity on the highways of the State of Tennessee shall hold the State of Tennessee, its officers and employees, harmless from any claims for damages resulting from the exercise of any of the privileges granted under the Special Permits issued for such overweigh and/or overdimensional movement, and to this end, shall carry liability insurance with an Insurer, acceptable to the Commissioner of Transportation, and shall furnish a certificate of insurance to the Commissioner of Transportation, in the amount of not less than three hundred thousand dollars ($300,000) for each claimant injured and one million dollars ($1,000,000) per occurrence.

(2) The certificate of insurance shall provide that the Insurer shall give, to the Department of Transportation of the State of Tennessee, written notice of intention to terminate said required insurance by certified mail, said termination to become effective thirty (30) days after receipt of said notice from the Insurer by the Department of Transportation."

It is recommended that this is taken care of prior to ordering truck permits. The process takes at least 24 hours to get the insurance certificate approved by the state.

Texas - "Minimum coverage of $500,000 combined single limit excluding cargo. Must have TX MCR# or bond on file to haul oversize/overweight loads. Loads exempt are manufactured housing, portable buildings, oil well equipment, unladen lift equipment. Must carry minimum insurance of $750/500/10 for all ICC carriers." This process can take up to 3 days to get your insurance approved, please have this taken care of prior to ordering truck permits. Wisconsin - "Insurance may be required on file. Limits for loads 12' wide, 16' high or 75' long and does not exceed
120,000 GVW - $50/100/25. All other loads are $100/300/100 or $600,000 CSL (Combined Single Limits). In addition $200,000 casualty insurance will be required for loads in excess of 16' wide or 100' long, unless $600,000 combined single life coverage is provided. Must carry minimum insurance of $750/500/10 for all ICC carriers." This can generally be taken care of on the same day as truck permit is ordered.

All other states only require the "minimum insurance of $750/500/10 for all ICC carriers." They do not need a certificate on file, but proof of insurance must be kept in the cab of the truck. All of the above requirements are subject to change and should be checked on with the state prior to ordering permits.
